Membership Term & Payment Method
The Personalized Healthcare annual fee does not count toward your medical insurance deductible. Some members are able to use their Health Savings Accounts or Flexible Spending Accounts, which allows annual fees to be paid with pre-tax dollars. However, you must check with your insurance benefits administrator to determine eligibility, as each plan varies and has their own policy regarding the use of these accounts.
New Members: All Personalized Healthcare memberships are for one full year term. A contract must be initiated and payment cleared in order for your membership to be executed. You can then make your first appointment. We accept major credit cards and Apple Pay. Personal checks are to be made payable to Daniel Gaitan Personalized Healthcare, LLC.
Member Renewals: For your convenience, we can send an invoice through the United States Postal Service or via email with an attached payment link. You may pay by using an accepted major credit card or Apple Pay through a secure portal. Personal checks are to be made payable to Daniel Gaitan Personalized Healthcare, LLC.
